ABSTRACT

OBJECTIVES@#To explore the cytotoxicity of four wild mushrooms involved in a case of Yunnan sudden unexplained death (YNSUD), to provide the experimental basis for prevention and treatment of YNSUD.@*METHODS@#Four kinds of wild mushrooms that were eaten by family members in this YNSUD incident were collected and identified by expert identification and gene sequencing. Raw extracts from four wild mushrooms were extracted by ultrasonic extraction to intervene HEK293 cells, and the mushrooms with obvious cytotoxicity were screened by Cell Counting Kit-8 (CCK-8). The selected wild mushrooms were prepared into three kinds of extracts, which were raw, boiled, and boiled followed by enzymolysis. HEK293 cells were intervened with these three extracts at different concentrations. The cytotoxicity was detected by CCK-8 combined with lactate dehydrogenase (LDH) Assay Kit, and the morphological changes of HEK293 cells were observed under an inverted phase contrast microscope.@*RESULTS@#Species identification indicated that the four wild mushrooms were Butyriboletus roseoflavus, Boletus edulis, Russula virescens and Amanita manginiana. Cytotoxicity was found only in Amanita manginiana. The raw extracts showed cytotoxicity at the mass concentration of 0.1 mg/mL, while the boiled extracts and the boiled followed by enzymolysis extracts showed obvious cytotoxicity at the mass concentration of 0.4 mg/mL and 0.7 mg/mL, respectively. In addition to the obvious decrease in the number of HEK293 cells, the number of synapses increased and the refraction of HEK293 cells was poor after the intervention of Amanita manginiana extracts.@*CONCLUSIONS@#The extracts of Amanita manginiana involved in this YNSUD case has obvious cytotoxicity, and some of its toxicity can be reduced by boiled and enzymolysis, but cannot be completely detoxicated. Therefore, the consumption of Amanita manginiana is potentially dangerous, and it may be one of the causes of the YNSUD.

ABSTRACT

Objective: To explore the clinical features of hepatocerebral mitochondrial DNA depletion syndrome (MDS). Methods: The clinical data of 6 hepatocerebral MDS patients diagnosed in the Jinshan Hospital of Fudan University from January 2012 to December 2019 were retrospectively collected and analyzed. Related literature published before January 2020 were searched with the key words of "DGUOK""MPV17""POLG""C10orf2" in PubMed, China national knowledge infrastructure (CNKI) and Wanfang database. Results: All the 6 hepatocerebral MDS cases were male. The age of onset ranged from 3 days to 8 months. The most common initial symptoms were cholestasis and developmental retrogression. The main clinical manifestations included hepatomegaly (4 cases), hypotonia (3 cases), growth retardation (4 cases), cholestasis (5 cases), coagulopathy (5 cases), hypoalbuminemia (3 cases), hypoglycemia (4 cases), hyperlactacidemia (5 cases), and abnormal blood metabolism screening (6 cases). The isotope hepatobiliary imaging revealed no gallbladder and intestinal tract development within 24 hours in 2 patients. Regarding the cranial imaging examination, the head CT found widening of the extracranial space in 1 case, the brain magnetic resonance imaging (MRI) found ventricular enlargement in 2 cases, and the brain ultrasound found peripheral white matter injury in 1 case. Two cases were lost to follow-up, one died of liver failure, and three died of multiple organ failure due to aggravated infection. Among the 6 cases, there were 3 with MPV17 variation (c.182T>C and c.279G>C were novel), 1 with POLG variation (c.2993G>A was novel), 1 with DGUOK variation (c.679G>A homozygous mutation, parthenogenetic diploid of chromosome 2) and 1 with C10orf2 variation (c.1186C>T and c.1504C>T were novel). The literature review found that 129, 100, 51 and 12 cases of hepatocerebral MDS were caused by DGUOK, MPV17, POLG and C10orf2 gene variations, respectively. And the most common clinical manifestations were liver dysfunction presented with cholestasis and elevated transaminase, metabolic disorders including hypoglycemia and hyperlactacidemia, and diverse neurologic symptoms including developmental retardation, hypotonia, epilepsy and peripheral neuropathy. Besides, 1/3 of the patients with C10orf2 variation developed renal tubular injury. Conclusions: Hepatocerebral MDS mainly present with liver dysfunction, metabolic disorder and neuromuscular impairment. Different genotypes show specific clinical manifestations.

ABSTRACT

Objective:To investigate the effect of intervention on oral health of pre-pregnancy women before and after oral health education. Methods:A total of 40 pre-pregnancy women were selected from the Reproductive Medicine Center of General Hospital of Ningxia Medical University according to the inclusion criteria, general conditions, clinical evaluation of plaque and oral health education. Their oral health conditions were evaluated before and after oral health intervention. Results:Based on the oral health status survey, there were significant differences between before and after intervention (all P<0.001) in the following five items: “bleeding from brushing teeth”, “difficulty biting or chewing food”, “sensitivity of teeth or gums to cold, hot, or sweet stimuli”, "restriction of the type and amount of food eaten for dental reasons” and “medication for oral pain or discomfort”. There were significant differences between before and after intervention (all P<0.001) in four items of oral health care behavior including “How often do you brush your teeth?”, “How do you brush your teeth?”, “gargle after meals”, and “floss use or not” but showed no significant difference in toothbrush replacement (P=0.467). There were significant differences (all P<0.001) in five items of oral health knowledge including “periodontal disease can lead to premature delivery of newborns”, “periodontal disease can lead to low birth weight of newborns”, “need oral examination before pregnancy”, “pregnancy prone to oral diseases”, “mid-pregnancy is the best period for the treatment of oral diseases”. The oral plaque index before intervention was 5.47±1.08 and reduced to 4.37±0.94 after intervention (t=7.93, P=0.001). Conclusion:Through education intervention, the oral health status of pre-pregnancy women can be improved. The knowledge of oral health can be improved and the level of oral health care can be enhanced. Oral health intervention can effectively reduce the level of plaque in pre-pregnancy women and improve the efficiency of plaque clearance.

ABSTRACT

Objective To explore the association of cardiac disease associated genetic variants and the high incidence of Yunnan sudden unexplained death (YNSUD) in Yi nationality. Methods The genomic DNA was extracted from peripheral blood samples collected from 205 Yi villagers from YNSUD aggregative villages (inpatient group) and 197 healthy Yi villagers from neighboring villages (control group). Fifty-two single nucleotide variants (SNVs) of 25 cardiac disease associated genes were genotyped using matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF-MS). The SPSS 17.0 was used to analyze data. The pathogenicities of variants with differences between the two groups that have statistical significance were predicted by protein function prediction software PolyPhen-2 and SIFT. All villagers from inpatient group were given electrocardiogram (ECG) examination using a 12-lead electrocardiograph. Results The allele frequency and the genotype frequency of missense mutation DSG2 (rs2278792, c.2318G>A, p.R773K) of pathogenic genes of arrhythmogenic right ventricular cardiomyopathy (ARVC) in inpatient group was higher than that in control group (P<0.05). Abnormal ECG changes were detected in 71 individuals (34.6%) in the inpatient group, among which 54 individuals carried R773K mutation, including clockwise (counterclockwise) rotation, left (right) axis deviation, ST segment and T wave alteration and heart-blocking. Conclusion Definite pathogenic mutations have not been found in the 52 cardiac disease genes associated SNVs detected in Yi nationality in regions with high incidence of YNSUD. The cause of high incidence of YNSUD in Yi nationality needs further study.

ABSTRACT

The purpose of this research is to investigate the effects of acacetin on serum lipid metabolism and atherosclerosis in mice and explore its molecular mechanism. HepG2 cells were treated with different concentrations of acacetin. The expression of LDL receptor (LDLR) and sterol-regulatory element binding protein-2 (SREBP-2) were detected by RT-qPCR and/or Western blot. C57BL/6J mice were given acacetin (50 mg·kg-1) for 5 weeks by gavage. Serum total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and triglyceride (TG) were analyzed by an automatic biochemical analyzer. The expression of LDLR or SREBP-2 was detected by Western blot. After 12 weeks of intragastric administration of acacetin (30 mg·kg-1) in apolipoprotein E knockout (ApoE KO) mice, the serum lipid levels were determined by an automatic biochemical analyzer. The lipid deposition in aortic plaque (en face) and aortic root plaque were stained with oil red O. The expression of LDLR and SREBP-2 were detected by RT-qPCR and/or Western blot. The intestinal content microflora was analyzed by 16S rDNA sequencing (All animal studies were approved by the Animal Experimentation Ethics Committee of Institute of Medicinal Biotechnology, CAMS & PUMC). In vitro results indicated that acacetin significantly up-regulated LDLR mRNA and protein levels, and stimulated LDLR transcription factor SREBP-2 protein expression. As indicated from in vivo studies, compared with control group, acacetin significantly decreased the serum levels of TC and LDL-C in C57BL/6J mice by 34% and 57% (P<0.01), respectively. Furthermore, mechanic study showed that acacetin significantly increased the protein expression of hepatic LDLR and SREBP-2. Although the results of serum lipid profiles, hepatic LDLR/SREBP-2 expression and area of atherosclerotic lesions in aorta and aortic root in ApoE KO mice showed differences between acacetin and high-fat diet group, the differences did not reach statistical significance. Nevertheless, acacetin exhibited a profound influence on the composition of the intestinal microbiota as indicated by 16s rDNA sequencing analysis. In conclusion, these results demonstrated that acacetin can decrease the serum lipid levels in C57BL/6J mice through up-regulation of hepatic LDLR and SREBP-2, and alter gut microflora in high-fat diet fed Apo KO mice. This study suggests the possibility that acacetin has a potential role in inhibiting the progression of atherosclerosis.

ABSTRACT

Nonalcoholic fatty liver disease ( NAFLD), a clinical metabolic stress-induced liver injury, is expressed as a feature of intrahepatic cell steatosis and excessive fatty deposition. NAFLD is a rising cause of common chronic liver disease worldwide. However, due to the lack of idealized NAFLD experimental models, the pathogenesis of NAFLD remains to be elucidated and consequently the research and development of therapeutic drugs against NAFLD has been making a relatively slow progress. Therefore, the establishment of cell and animal models is of great significance for exploring its pathogenesis and screening relevant experimental drugs. In this paper, the experimental models involved in NAFLD are reviewed and its advantages and disadvantages are evaluated further in vitro and in vivo, which provides a sufficient theoretical basis and model selection for the investigation and development of innovative drugs against NAFLD.

ABSTRACT

This paper aimed to study the six chemical components of Polygoni Multiflori Radix (gallic acid, quercetin, luteolin, kaempferol, resveratrol, apigenin). By the established pregnane X receptor (human pregnant X receptor, PXR) CYP3A4 mediated drug induced rapid screening technique, the effect of chemical components on the cell activity was detected by MTS cell method, and the value of IC₅₀ was calculated. The dual luciferase reporter system was used to co-transfect PXR reporter gene expression vector containing transcriptional regulation and CYP3A4 with HepG2 cells, with 10 μmol·L⁻¹ rifampicin (RIF) as a positive control, and 10 μmol·L⁻¹ of ketoconazole (TKZ) as negative control. Gallic acid, quercetin, luteolin, kaempferol, apigenin, resveratrol(5, 10, 20 μmol·L⁻¹) were used to incubate for 24 h, and the luciferase activity was detected. The results showed that when plasmid pcDNA3.1 was co-transfected with pGL4.17-CYP3A4, gallic acid and resveratrol had an inhibitory effect on the regulation of CYP3A4, and quercetin, luteolin, kaempferol had an inductive effect on CYP3A4; when pcDNA3.14-PXR was co-transfected with pGL4.17-CYP3A4, quercetin, luteolin, kaempferol, apigenin, resveratrol had an inductive effect. To sum up, the 6 reported liver injury components had inhibitory or activating effects on CYP3A4. After PXR plasmid was involved, 5 components had an inductive effect on CYP3A4, and the inductive effects of 2 components were significantly different. In this experiment, we found that 2 kinds of potential liver injury components in Polygoni Multiflori Radix had been induced by CYP3A4, which was achieved through PXR regulation. It suggested that attention shall be paid to potential drug interactions when combined with Polygoni Multiflori Radix, so as to improve the safety and efficacy.

ABSTRACT

Nanopore technique is a low-cost tool for single-molecule level analysis without the need of label or amplification. The solid nanopores have been widely used in many fields such as chemistry and life sciences due to their advantages such as high stability, good tolerability, controllable size, and easy for modification. The solid nanopores commonly used in recent years are fabricated using two types of materials: membrane and tube. The membrane materials include silicon nitride, two-dimensional materials, aluminium oxide, and polymer membranes. The tube materials mainly include glass capillary and carbon nanotube. This review summarizes and prospects the research progress of different solid nanopores.

ABSTRACT

Objective To explore the prevalence and the influencing factors of osteoporosis among elderly residents in Lujiazui Community and to provide the reference for the prevention of osteoporosis.Methods The cluster sampling was used to select 1956 community residents aged 65 and above for investigation, which was performed with the questionnaire, physical examination and bone mineral density detection. Logistic regression analysis was applied to analyze the influencing factors of osteoporosis. Results The prevalence rate of osteoporosis of 1576 respondents was 35.41%. The prevalence rate in females (46.89%) was higher than males (16.47%) (P<0.01), Logistic regression analysis showed that weight (OR=0.05-0.22), central obesity (OR=5.19, 95%CI: 2.60-1.38), exercise (OR=0.12-0.43), diet (OR=0.34-0.38), drink (ORusual=4.19、 ORdaily=2.86) were the main influencing factors in males.Weight (OR=0.33-0.37), exercise (OR=0.34-0.57), diet (OR=0.25-0.59), diabetes history (OR=1.45, 95%CI: 1.09-1.92), hypertension history (OR=1.39, 95%CI: 1.07-1.82) were the main influencing factors in females. Conclusion The prevalence rate of osteoporosis of residents aged 65 and above in Lujiazui Community was 35.41% , with higher rate in females. Weight control, more exercise and non-vegitarian diets could lower the risk of osteoporosis .

ABSTRACT

The rapid screening technology was used to investigate the transcriptional regulation effect of main chemical constituents in tubers of Polygonum multiflorum, including 2,3,5,4'-tetrahydroxystilbene-2-O-β-D-glucopyranoside(THSG) and anthraquinones (such as rhein, chrysophanol, aloe-emodin, emodin) on CYP3A4 drug inducers induced by human pregnancy X receptor (PXR).The effect of chemical composition on the cell activity was detected by MTS cell viability assay. IC₅₀ was calculated. The expression vector and the reporter vector were co-transfected into HepG2 cells, with 10 μmol•L⁻¹ rifampicin (RIF) as a positive control, and 10 μmol•L⁻¹ ketoconazole (TKZ) as a negative control. After treated with different concentrations of anthraquinones (2.5, 5, 10 μmol•L⁻¹) for 24 h, the cells were tested for dual luciferase activity. The results show that the inhibitory effect of THSG, chrysophanol, emodin, rhein and aloe-emodin on CYP3A4 was inhibited by co-transfection of pcDNA3.1 and pGL4.17-CYP3A4. The expressions of pcDNA3.14-PXR and pGL4.17-CYP3A4 were induced by the four compounds. Besides, emodin had a direct inducing effect. In conclusion, the four anthraquinone compounds have an inducing effect on CYP3A4 by PXR, but emodin can directly induce CYP3A4. THSG can inhibit CYP3A4, but plasmid can induce CYP3A4 after intervened with PXR.These results suggest that we should pay attention to the liver function and avoid liver damage in the combined administration of drugs.

ABSTRACT

To investigate the effect of clinical dose of Realgar-Indigo Naturais formula (RIF) and large-dose of Realgar on main drug-metabolizing enzymes CYP450s of rat liver, as well as its regulatory effect on mRNA expression. Wistar rats were administrated orally with tested drugs for 14 days. A Cocktail method combined with HPLC-MS/MS was used in the determination of 4 cytochrome P450 isozymes (CYP1A2, CYP2B, CYP3A and CYP2C) in liver of the rats, and the mRNA expression levels of the above subtypes were detected by real-time fluorescent quantitative PCR. The results showed that RIF can significantly induce CYP1A2 and CYP2B enzyme activity, and inhibit CYP3A enzyme activity. This result was consistent with the mRNA expression. However, its single compound showed weaker or even contrary phenomenon. Different doses of Realgar also showed significant inconsistencies on CYP450 enzymes activity and mRNA expression. These phenomena may be relevant with RIF compatibility synergies or toxicity reduction. The results can also prompt drug interactions when RIF is combined with other medicines in application.

To apply the ultrasonic 3D reconstruction method to measure the anatomic image data of lacrimal sac in the normal adult Chinese population, providing imaging anatomical basis for the clinical diagnosis and treatment of lacrimal duct diseases. 80 volunteers without lacrimal duct diseases were enrolled. The GE Voluson E8 ultrasonic diagnostic apparatus was used, with gynecological intracavitary probe (frequency: 5 ~ 12MHz) for the examination and measurement. Each datum was repeated three times for the average value. Among the 80 eyes, the lacrimal sacs of 74 eyes exhibited the clear imaging, with the imaging rate as 92.5 %. the vertical diameter of lacrimal sac cavity (LSC): 13.3±2.2 mm; anteroposterior diameter: 6.0±1.1 mm; trans diameter: 4.9±0.9 mm; the vertical distance from the sac cavity top to the skin surface: 6.3±0.12 mm, the vertical distance from the sac cavity bottom to the skin surface: 6.5±0.11 mm; the distance from the angular artery to the medial canthus: 7.8±0.05 mm; the vertical distance from the angular artery to the skin surface: 7.7±0.08 mm. The lacrimal sac and angular artery in the adults could be shown clearly in ultrasonic 3D reconstruction, the accumulated anatomical parameters could provide the instructive meanings towards the clinical diagnosis and treatment of lacrimal diseases.


El objetivo de este estudio consistió en aplicar el método de reconstrucción 3D de ultrasonidos para medir los datos de las imágenes anatómicas del saco lagrimal en una población china adulta normal, proporcionando imágenes con base anatómica para el diagnóstico clínico y el tratamiento de las enfermedades de los conductos lagrimales. La muestra consistió en 80 voluntarios sin enfermedades de los conductos lagrimales. Se utilizó el aparato de ultrasonido GE Voluson E8, con una sonda intracavitaria ginecológica (frecuencia: 5 ~ 12 MHz) para realizar el examen y la medición. Cada dato se repitió tres veces para obtener el valor medio. De los 80 ojos, los sacos lagrimales de 74 ojos mostraron una imagen clara, siendo la tasa de formación de imágenes del 92,5 %. Se realizaron mediciones: diámetro vertical de la cavidad del saco lagrimal (LSC): 13,3 ± 2,2 mm; diámetro anteroposterior: 6,0 ± 1,1 mm; diámetro transversal: 4,9 ± 0,9 mm; distancia vertical desde la porción superior de la cavidad del saco lagrimal hasta la superficie de la piel: 6,3 ± 0,12 mm, distancia vertical desde el fondo de la cavidad del saco lagrimal hasta la superficie de la piel: 6,5 ± 0,11 mm; distancia desde la arteria angular al canto medial: 7,8 ± 0,05 mm; distancia vertical desde la arteria angular a la superficie de la piel: 7,7 ± 0,08 mm. El saco lagrimal y la arteria angular en los adultos se podría demostrar claramente en la reconstrucción 3D con ultrasonido. Los parámetros anatómicos acumulados podrían ser de significativo valor para un certero diagnóstico clínico y tratamiento de enfermedades de los sacos lagrimales.

ABSTRACT

Objective To investigate the relationship between phenomenon of aggregation on multiple metabolic abnormalities and their family history.Methods A random cluster sampling was carried out to study the phenomenon of multiple metabolic abnormalities among the adult inhabitants in a county of Henan province.Questionnaire,physical examination and biochemical tests were admitted.Multiple correspondence analyses were used to explore the aggregation of multiple metabolic abnormalities among objects including hyperlipidemia,diabetes,coronary heart disease etc.Corresponding relationships between objects and their parents on multiple metabolic abnormalities were also analyzed.Results In total,3901 people were investigated including 1428 (36.6%) males and 2473 (63.4%) females.The mean age of them was 32.8 ± 6.3.Data showed that the Cronbach' s α were 0.547 and 0.387 while Eigen values were 1.971 and 1.535 in two dimensions,regarding the correspondence analysis on their parents' multiple metabolic abnormalities.Cronbach' s α levels were 0.598 and 0.457 together with Eigen values as 2.263 and 1.743 in two dimensions on the correspondence analysis of the objects' and their parents' multiple metabolic abnormalities.Results from Multiple correspondence analysis diagrams showed that there was an aggregation of varietymetabolic abnormalities in both objects and their parents but not between objects and their parents or between fathers and mothers,though x2 test showed a weak correlation between some of the categories.The ranges of contingency coefficients between metabolic abnormalities were 0.04 to 0.11,0.04 to 0.08 and 0.04 to 0.11 between parents,objects and fathers,objects and mothers,respectively.When compared with simple obesity,the central obesity aggregated appeared more obviously with other metabolic abnormalities.Conclusion Both objects and their parents showed an aggregation of variety metabolic abnormalities.Aggregation of central obesity showed more obvious on other metabolic abnormalities than the simple obesity.Family histories of metabolic abnormalities played a moderate role in the generations suffering from multiple metabolic abnormalities.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of triptolide on airway remodeling and the expression of nuclear factor-kappaB, Bcl-2 in asthmatic rats.</p><p><b>METHODS</b>40 rats were randomly divided into 5 groups (n = 8): (1) Control group; (2) Asthmatic 4 week group; (3) Asthmatic 6 week group; (4) Therapeutic 4 week group; (5) Therapeutic 6 week group. The airway resistance and eosinophilic inflammation of airway wall were observed. The airway wall thickness (WA/Pi), the bronchial smooth muscle thickness (smooth muscle area/Pi) and the number of bronchial smooth muscle nucleus (N/Pi) were measured by image analysis system. The expression of PCNA, nuclear factor-kappaB and Bcl-2 protein were determined by immunohistochemical staining and Western blot. The expression of Bcl-2 mRNA was determined by reverse transcription-polymerase chain reaction(RT-PCR).</p><p><b>RESULTS</b>(1) The expression of NF-kappaB protein in asthmatic 4 week group and asthmatic 6 week group was significantly higher than that in control group, respectively (P < 0.01). The above-mentioned parameters of therapeutic 6 week group were significantly lower than those of asthmatic 4 week group, asthmatic 6 week group and therapeutic 4 week group, respectively (P < 0.01, P < 0.01 P < 0.05). (2) The expression of Bcl-2 protein and mRNA of asthmatic 4 week group and asthmatic 6 week group were significantly higher than those in control group respectively (P < 0.01). The expression of Bcl-2 protein of therapeutic 6 week group was significantly lower than those of asthmatic 4 week group, asthmatic 6 week group and therapeutic 4 week group respectively (P < 0.05, P < 0.01, P < 0.01), but the expression of Bcl-2 mRNA was significantly higher than the above-mentioned groups respectively (P < 0.01), the expression of Bcl-2 protein and mRNA of therapeutic 6 week group were higher than control group respectively (P < 0.05, P < 0.01). (3) The expression of PCNA protein of asthmatic 4 week group and asthmatic 6 week group were significantly higher than those of control group respectively (P < 0.01). (4) The WA/ Pi, the smooth muscle area/Pi and the N/Pi of asthmatic 4 week group and asthmatic 6 week group were significantly higher than those of control group, respectively (P < 0.01). The above-mentioned parameters of therapeutic 6 week group were significantly lower than those of asthmatic 4 week group, asthmatic 6 week group and therapeutic 4 week group, respectively (P < 0.01). (5) The airway resistance of asthmatic 4 week group and asthmatic 6 week group were significantly higher than those of the control group, respectively (P < 0.01). The above-mentioned parameters of therapeutic 6 week group were significantly lower than those of asthmatic 4 week group, asthmatic 6 week group and therapeutic 4 week group, respectively (P < 0.01, P < 0.01, P < 0.05).</p><p><b>CONCLUSION</b>The proliferation of airway smooth muscle(ASM) is related with apoptosis of airway smooth muscle cells in asthma. NF-kappaB may be involved in the process. Triptolide may prevent apoptosis of ASMCs and decrease the proliferation of ASM by inhibiting the expression of NF-kappaB, Bcl-2.</p>

ABSTRACT

<p><b>BACKGROUND AND OBJECTIVE</b>CT-guided microwave coagulation is a minimally invasive surgery for patients with liver cancer. Total intravenous anesthesia with propofol and fentanyl is commonly used. The depth of anesthesia during microwave coagulation for liver cancer is still monitored by clinical signs. There are few subjective and effective indicators. This study explored the application of Narcotrend-assisted "depth of anesthesia" monitoring on microwave coagulation for patients with liver cancer during total intravenous anesthesia with propofol and fentanyl.</p><p><b>METHODS</b>Forty liver cancer patients underwent CT-guided microwave coagulation were randomly assigned to receive Narcotrend index monitoring or standard clinical monitoring for depth of anesthesia with 20 patients in each group. All patients received total intravenous anesthesia with propofol and fentanyl. The depth of anesthesia for patients in the Narcotrend group was measured according to a Narcotrend index, which was maintained between D2 and E0. The depth of anesthesia for those in the standard clinical practice group was measured according to heart rate, mean arterial pressure, and patient movement. Changes of hemodynamics, the duration of the emergence from anesthesia, and the recovery of orientation were recorded. The doses of propofol and fentanyl, postoperative visual analogue scores (VAS), and the incidence of postoperative nausea and vomiting were also recorded.</p><p><b>RESULTS</b>There was no significant alteration in heart rate or mean arterial pressure between the two groups. Compared with other anesthetic stages, both heart rate and mean arterial pressure decreased during the induction of the anesthesia in the two groups(P<0.05). The doses of propofol were higher in the standard clinical practice group than in the Narcotrend group [(460+/-30) mg vs. (380+/-35) mg, P<0.01]. The duration of emergence and orientation were longer in the standard clinical practice group than in the Narcotrend group [(9.5+/-2.9) min vs. (4.9+/-2.2) min, P<0.01; (12.2+/-3.5) min vs. (6.6+/-3.2) min, P<0.01, respectively]. There was no difference in the dosage of fentanyl, VAS, or the incidence of postoperative nausea or vomiting between the two groups (P>0.05).</p><p><b>CONCLUSION</b>For patients with liver cancer, monitoring the depth of anesthesia with Narcotrend on microwave coagulation can contribute to lower dosage of propofol and shorten duration of recovery during total intravenous anesthesia with propofol and fentanyl.</p>

ABSTRACT

Objective To analyze the region cluster and risk factors of hypertension in the Chinese adult population and to explore the application of multilevel regression model in the risk factors of hypertension. Methods Multi-stage random sampling technique was used to choose 15 540 individuals aged 35-74 years from 10 regions in China. Two-level logistic regression models were fitted under MLwiN 2.02 software. Results The region cluster of hypertension existed and variance portion coefficient was 3.1%. After adjusting for the age and gender, overall obese people (BMI≥28 kg/m2) were 4.50(95%CI: 4.00-5.06) times, overweight people (BMI=24-27.9 kg/m2) were 2.26 (95%CI: 2.07-2.46) times more likely to be hypertensive as compared with those of normal BMI (18.5-23.9 kg/m2), and those centrally obesive people (Waist circumference≥85 cm in male or 80 cm in female) were 2.62 (95%CI: 2.42-2.83) times more likely to be hypertensive as compared with those of normal WC. The age-and gender-adjusted odds ratios (Ors) of triglyceride (TG), serum total cholesterol (TC), glucose, low-density lipoprotein cholesterol (LDL-C) , high-density lipoprotein cholesterol (HDL-C) and drinking alcohol were 2.10 (95% CI: 1.89-2.33) , 2.08 (95% CI: 1.84-2.35) , 1.85 (95% CI: 1.60-2.14) , 1.58 (95% CI: 1.38-1.81), 1.49(95%CI: 1.32-1.69) and 1.15(95%CI: 1.05-1.27), respectively. Conclusion The prevalence of hypertension was not only affected by individual risk factors, such as obesity, drinking alcohol, abnormal glucose and serum lipids profile, but also affected by the geographic environment where people resided in. Population-and risk factors targeted strategies, proved a promising way to reduce individual risk of hypertension in the primary prevention of hypertension.

ABSTRACT

Objective To define the stages of medical rescue after earthquake or tsunami disaster and their significance.Method Three stages of medical rescue were defined according to the clinical features of diseases in 3-37 days after earthquake or tsunami in Pakistan and Indonesia.The classification of diseases was based on the criteria of disaster medical rescue set by World Health Organization.Five diseases including injuries/wounds,acute upper respiratory tract infection,acute hemorrhagic enterocolitis,acute enterecolitis and other diseases of internal medicine except epidemic or suspected epidemic diseases were employed to define the stages of disaster medical rescue.Results Injuries/wounds mainly occurred in 6 days after disaster.The incidences were 61.45% to 79.52% and 61.48% to 72.35% in Pakistan and Indonesia,respectively.The incidence of acute upper respiratory tract infection reached to a considerably high rate one week after disaster.Other diseases usually appeared one week later with incidence of 33.93% to 71.11% and 31.50% to 52.11% in Pakistan and Indonesia,respectively.The rate of acute hemorrhagic enterocolitis was 2%o in Indonesia whereas 35‰of acute enterocolitis was found in Pakistan.Conclusion Three stages of disaster medical rescue could be well defined.(1)Early or critical stage(in 6 days after disaster):Treatment of injuries/wound and resuscitation was the primary step to deal with.(2)Intermediate of subaeute stage(7-30days following disaster):The incidences of various kinds of diseases increased and more medical resources were needed in order to treat the upper respiratory tract infection and to lower the disable rate,and to prevent the prevalence of epidemic diseases.(3)Later or recovery stage(1-3months after disaster):The general condition of victims became even better than that of those affected by commonly occurred local diseases,and during this period,there were shortages of medical facilities and medical personnel.Therefore,supplement of medical resources was required.The well defined stages of medical rescue have an important significance for medical rescue teams to treat victims and prevent epidemic diseases more efficiently and timely,and also provide a guidance of scientific basis for the logistics officers of local government as well as our government to make a proper and reasonable distribution of medical resources,and dispatch and arrangement of medical rescue teams.This guidance also helps to make victims re- reliant.

ABSTRACT

<p><b>OBJECTIVE</b>To understand the incidence of post-stroke depression (PSD) and the therapeutic effect of acupuncture-moxibustion.</p><p><b>METHODS</b>Five hundred and sixty cases were rating with Hamilton depression scale, and lesion parts, gender, age and property of stroke between the PSD and non-PSD were compared by analysis of variance, chi2 test, and then the patients of PSD were randomly divided into two groups and treated respectively with Prozac-20 and acup-moxibustion at Sishencong (EX-HN 1), Anmian, Neiguan (PC 6), Shenmen (HT 7), Zusanli (ST 36), Sanyinjiao (SP 6), Taichong (LR 3), Zhaohai (KI 6), Shenmai (BL 62).</p><p><b>RESULTS</b>The total incidence rate of PSD was 43.9%, with no relation to the lesion parts, gender, age and property of stroke (P > 0.05), and there were very significant differences in HAMD scores before and after treatment in the two groups (P < 0.0001), with no significant difference in the effective rate between the two groups (P > 0.05).</p><p><b>CONCLUSION</b>PSD has a high incidence and influences the recovery of nervous function, and it should be treated at early stage. Acup-moxibustion and Prozac-20 have similar therapeutic effect.</p>

ABSTRACT

It was widely believed that the interaction between rice and rice blast fungus can be interpreted by the gene-for-gene hypothesis. Two interaction models between rice blast fungus and anti-disease genes had been briefed. They were receptor-ligand model and guard model. The progress of research about molecular marker and position on avirulence genes of the pathogens of rice blast (Magnaporthe oryzae) was reviewed, and the methods to clone and the cloned avirulence genes of rice blast fungus were also summarized.
